The scene opens with Sam laid out on a bed. Dean is watching him, although it is obvious Sam is dead. Bobby walks in, telling Dean he needs to eat. He then suggests it’s time they bury Sam but Dean refuses. Bobby asks Dean to go with him because something “end of the world big is going down,” but Dean refuses saying to let it end and tells Bobby to leave;which he does.
Jake has escaped Cold Oak and is camping out. The YED appears to him and congratulates him, he is the last man standing. Jake wants to know what the demon wants him for; he says he’s going to kill the demon. The demon threatens Jake’s family and Jake realises he has no choice but to say he’ll do the demon’s bidding.
Dean sits alone in the abandoned house, still watching Sam’s lifeless body. He talks about when they were little; about when Sam was five and had just started asking questions like where was mom. Dean says he always tried to protect Sam and keep him safe, because he felt it was his responsibility. Except now he’s screwed that job up. Dean says he is sorry and that he always lets down the people he loves. He keeps saying what is he supposed to do, before rushing out to the Impala.
Speeding to the nearest crossroads, Dean then puts a box with his picture and other items at the center of the cross section, waiting to summon the “crossroads demon.” When she appears Dean tells her he wants to make a deal for Sam’s life.
Dean offers his soul for Sam back and ten years of life. The demon says no way. He offers 9 then 8 years of life, but the demon offers Sam back and just one year to live. Dean has no choice but to accept. The demon makes a clause, saying if Dean ever tries to get out of the deal then Sam will die again. Dean kisses the demon, sealing the pact.
Sam wakes up, confused. He goes to a mirror and tries to look at his back where he was stabbed. He knows something is wrong, but can’t figure out what. Dean walks in and is so relieved to see his brother back among the living, he can’t help but give Sam a hug.
Sam asks what happened, Dean lies, saying Sam was stabbed in the back but Bobby patched him up. Dean asks who the kid was and Sam tells him he is called Jake. Sam asks if Dean got Jake, but Dean says no, he got away.
Sam and Dean get something to eat. Sam tells his brother everything that happened in Cold Oak, and that the demon only wanted one 'special child'. He can’t wrk out why he and Jake are both still alive as that is two kids. Dean brushes this aside, saying Sam was left for dead. Sam seems to accept this but says they have to figure out what the demon wanted the special kid for, and stop him. Dean wants Sam to rest first, but Sam refuses and says they should call the Roadhouse. Dean has to tell him the Roadhouse was burned down because of something Ash found out and that Bobby is trying to find out what that something was. Sam insists they go to Bobby’s, despite Dean’s protests that he should rest.
Bobby is shocked when he opens the door to see Sam alive. Although he doesn’t say anything, Sam notices the strange reaction. The brothers go inside and Bobby shows them what he’s found. He doesn’t know what it means, but demonic omens have skyrocketed in a certain area, all except for one place in the centre. He shows them on a map. One place in Southern Wyoming is totally clean, as if demons are surrounding it, but can’t get inside. Bobby asks Sam to take a look for clues while he and Dean go outside to get more research books.
Outside, Bobby confronts Dean about what he’s done for Sam. Bobby is furious when Dean confesses, saying he has made a deal, but he shouldn’t have been alive anyway after the crash. Bobby asks if he has that low an opinion of himself. Dean says he couldn't let Sam die, he's his little brother. Bobby asks how does he think Sam will fell knowing Dean will go to hell. Dean begs Bobby not to tell Sam, but then the pair are distracted by a noise in the yard - it’s Ellen!
Bobby asks Ellen to drink some holy water to prove she’s not a demon or possessed. She knocks back the water then asks for whiskey. Dean asks what happened at the roadhouse and Ellen explains she had to go out for supplies. While she was out Ash called her and told her to look in the safe, but by the time she arrived back the Roadhouse was on fire and everyone dead. Ellen managed to get to the safe as it was in the basement. Inside the safe was another map of Wyoming.
Bobby figures out that the marks on the map are all abandoned frontier churches all built by Samual Colt. Private railway lines connect each church and when the lines are drawn out they make up a huge, iron Devil’s Trap - demons can’t cross the iron lines. In the middle of the Devil’s Trap is an old cowboy cemetery. Sam wonders what Samual Colt was trying to protect there and Dean suggests Colt could have been trying to keep something in? Sam asks Bobby if the demons could get inside the trap and Bobby says that no full blood demon could. Sam realizes what the demon wanted the 'special child' for – to get inside the Devil’s Trap.
Jake drives up and is met by the demon at the edge of the Colt railway line. The demon tells Jake that 15 miles in is a cemetery and a crypt. Jake has to open the crypt because he can’t go in there himself. Jake says no, but the demon gives him the Colt and explains it’s the only gun that can kill him. Jake points the gun at the demon, but the demon laughs and offers Jake and his family a better world, where Jake will be royalty. It’s too tempting and Jake concedes.
Night at the cowboy cemetery. Jake walks among the graves towards the crypt. Dean, Sam, Ellen and Bobby appear from hiding places and pull their guns on him. Jake looks at Sam shocked, saying Sam was dead, he made sure of that when he cut through his spinal cord. Sam is clearly surprised and unnerved. Jake laughs and makes Ellen put her own gun to her head, just by suggesting it. He tells the others that once you give into the abilities there are lots of "Jedi mind tricks" they can do. He says they all should put their guns down and then he turns, placing the Colt in a strange rotating lock on the crypt.
Sam manages to get his gun back and repeatedly shoots Jake; even Dean flinches. However they are unaware the lock is opening, when they notice Dean tugs the Colt from the spinning lock and they take cover as literally “All Hell Breaks Loose.” The iron train lines are broken by the force and the demon can finally enter the cemetery.
Bobby, Ellen and Sam try to close the gate as Dean spots the YED and tries to shoot him. The YED takes the gun away using his powers and it flies into his hand. He tosses Dean into a headstone and pins him there. Sam sees what’s happening and runs for his brother, but the demon pins him to a tree.
The YED then proceeds to thank Dean for bringing Sam back, because Sam was his favourite. He also warns Dean that the deal may not have been as good as it seemed. He asks Dean if he is certain that the Sam he brought back is the same Sam he knows. He did just shoot Jake in cold blood. The YED mocks Dean one more time then draws the Colt to shoot him. Suddenly, John’s spirit appears from behind and fights the YED. The YED drops the gun and Dean is able to grab it, finally shooting the thing that they have been hunting for so long.
The YED drops to the floor and behind them Bobby and Ellen finally manage to close the hell gate. John’s spirit walks forward and places a hand on Dean’s shoulder. He nods to Sam too, tears in his eyes, but a smile on his face, and then he vanishes in a brilliant white light.
The brothers look over the YED's body and Sam says he just doesn’t know what to say. He asks Dean if he thinks their dad really climbed out of hell, Dean replies well the door was open, and he was stubborn enough. Sam says he can’t believe it’s over. Dean kneels by the YED, saying, “that was for our mom, you son of a bitch” to the demon’s shell.
Back at the Impala, Sam confronts Dean about what really happened, he'd noticed Jake looked at him like he'd seen a ghost. He asks if he died and Dean made a deal. Dean’s silence is as good as a confession. Sam tells Dean he shouldn’t have, but Dean insists he had to, it’s his job. Sam says his job is to protect Dean too. Sam tells Dean that he is his big brother, there is nothing he won't do for him and that he will save Dean for a change. Dean just smiles.
Bobby and Ellen walk up and Ellen says that loads of demons escaped through the gate. Sam thinks several hundred escaped – a whole army! Bobby nods, saying he hopes the boys are ready, because the war has just begun.... Dean grins and we see the boys open the Impala’s trunk. Dean tosses in the Colt and says “We got work to do.” The trunk lid closes to darkness.
